In preparation to petition the court under G.S. 108A‑106(f), documentation must be made of specific information indicating that:

1. (1) the adult lacks the capacity to consent;
2. (2) the adult is in need of protective services;
3. (3) the adult is being financially exploited; and
4. (4) no one else is able or willing to arrange for protective services.
