The evaluation must determine:

1. (1) whether or not the adult is disabled in accordance with the statutory definition in G.S. 108A‑101(d);
2. (2) whether or not the adult is abused, neglected or exploited as defined in G.S. 108A‑101(a)(j) or (m);
3. (3) whether or not the adult is in need of protective services as defined in G.S. 108A‑101(e);
4. (4) whether or not the adult lacks the capacity to consent to protective services.
