1. (a) The governing body shall maintain an application for each provider of private home respite which includes the following:

   1. (1) identifying information;
   2. (2) preference of time when respite care can be provided;
   3. (3) age and gender preference of respite clients.
2. (b) Only the respite program director or his designee shall arrange respite care between the client's family and the respite provider.
