1. (a) The Department shall make available, to the DHHS review team, a written annual report summarizing the activities and recommendations of the quality assurance committee.

2. (b) This report shall include, at a minimum, the following functional areas:

   1. (1) client care evaluation studies;
   2. (2) client record reviews;
   3. (3) utilization and peer reviews;
   4. (4) clinical supervision;
   5. (5) employee education and training activities; and
   6. (6) the results of program evaluation.
