An applicant proposing to develop a new Medicare-certified home health agency pursuant to a need determination in the annual State Medical Facilities Plan in effect as of the first day of the review period shall:

1. (1) provide projected utilization for each of the first three full fiscal years of operation following completion of the project;
2. (2) project to serve at least 325 residents of the proposed service area during the third full fiscal year of operation following completion of the project; and
3. (3) provide the assumptions and methodology used to provide the projected utilization required in Item (1) of this Rule.
