Licenses shall remain in effect up to 12 months, unless any of the following occurs:

1. (1) Department imposes an administrative sanction which specifies license expiration;
2. (2) closure;
3. (3) change of ownership;
4. (4) change of site;
5. (5) change in bed complement; or
6. (6) failure to comply with Rule .2104 of this Section.
