1. (a) Form PP1 is the permit issued for articles contained in a shipment which meets the requirements of all North Carolina plant pest quarantines. The following information is required:

   1. (1) date issued;
   2. (2) void after;
   3. (3) shipped by;
   4. (4) shipped from;
   5. (5) consigned to;
   6. (6) vehicle identification.
2. (b) Issuance of the permit requires the quantity and article identification.
3. (c) The permit must be signed by an inspector.
