The contingency plan shall be reviewed and immediately amended and resubmitted to the Board, if necessary, whenever:

1. (1) the plan fails in an emergency;
2. (2) the facility changes ‑‑ in its design, construction, operation, or maintenance;
3. (3) the list of alternate changes; or
4. (4) the list of emergency equipment changes.
