(1) A person may receive services provided for under this part if the department determines that:
- (a) the person has a severe disability;
- (b) the person lacks social competence, mobility, experience, skills, training, or other characteristics of independence;
- (c) the person is in need of services provided for under this part; and
- (d) the needed services are available for that person.
- (2) The receipt of services under this part by an eligible person shall be governed by an individual service plan developed in accordance with state and federal requirements.
