1. (1) The department of justice must provide a common reporting form for the report required under 50-32-401.

2. (2) The form must contain the following information:

   1. (a) name of the substance;
   2. (b) quantity of the substance sold, transferred, or furnished;
   3. (c) the date the substance was sold, transferred, or furnished;
   4. (d) the name and address of the person buying or receiving the substance; and
   5. (e) the name and address of the manufacturer, wholesaler, or retailer.
