(1)
(a) The secretary of state may prescribe and furnish on request forms for:
- (i) an application for a certificate of existence or certificate of registration;
- (ii) a foreign corporation's registration statement;
- (iii) a foreign corporation's statement of withdrawal;
- (iv) a foreign corporation's transfer of registration statement; and
- (v) the annual report.
- (b) If the secretary of state requires, use of these forms is mandatory.
- (2) The secretary of state may prescribe and furnish on request forms for other documents required or permitted to be filed by this chapter, but their use is not mandatory.
