"Home health care" means services provided by a licensed home health agency to an insured in the insured's place of residence that is prescribed by the insured's attending physician as part of a written plan of care. Services provided by home health care include:

1. (1) nursing;
2. (2) home health aide services;
3. (3) physical therapy;
4. (4) occupational therapy;
5. (5) speech therapy;
6. (6) hospice service;
7. (7) medical supplies and equipment suitable for use in the home; and
8. (8) medically necessary personal hygiene, grooming, and dietary assistance.
