1. (1) An insurer may offer health plans with a limited geographic area to individuals who live or work in that area, as long as the insurer meets the network adequacy requirements set forth in these rules and provides the commissioner with the following information:

   1. (a) a description of the geographic area; and
   2. (b) the rationale for limiting the sales of that product to that area.
2. (2) All insurers who are licensed as a health service corporation and all insurers who are qualified health plan issuers in the federally facilitated exchange must offer a health plan in all parts of the state.
