1. (1) Standard plans offered by HMOs must comply with ARM 6.6.5008. HMO plans may require that all services provided in ARM 6.6.5008 must be rendered or referred by a primary care provider.
2. (2) Standard plans offered by HMOs must offer a comparable level of benefits to a standard plan contemplated in 33-22-1828 , MCA and ARM 6.6.5008 as determined by the benefit value.
