1. (1) A complaint shall contain the following:

   1. (a) the full name, address, and telephone number, if any, of the person making the complaint (complainant) ;
   2. (b) the full name, address, and telephone number, if any, of the person against whom the complaint is made (respondent) ;
   3. (c) a clear and concise statement of the facts(s) , including pertinent dates(s) constituting the alleged violation(s) of the ethics code;
   4. (d) the specific provision of the ethics code which is alleged to have been violated;
   5. (e) the verified signature of the complainant swearing or affirming that the charge is true.
2. (2) A complaint may be filed on a form available on request from the commissioner's office.
