1. (1) The letter of a radio common carrier transmitting a proposed new rate schedule to the commission shall:

   1. (a) include a brief description of the proposed changes in service, rate, or charge; and
   2. (b) name an employee of the carrier who shall be responsible for answering questions concerning the application.
