1. (1) If a licensed or registered child care provider employs staff, the provider must maintain the following records for each staff member:

   1. (a) contact information; and

   2. (b) acknowledgment of, and agreement to follow, program policies for:

      1. (i) identifying and preventing shaken baby syndrome and abusive head trauma;
      2. (ii) mandatory reporting; and
      3. (iii) safe sleep, if the program provides care for children under two years old.
