1. (1) When the facility anticipates discharge, a resident must have a discharge summary that includes:

   1. (a) a recapitulation of the resident's stay;

   2. (b) a final summary of the resident's status which includes:

      1. (i) medically defined conditions and prior medical history;
      2. (ii) medical status measurement;
      3. (iii) physical and mental functional status;
      4. (iv) sensory and physical impairments;
      5. (v) nutritional status and requirements;
      6. (vi) special treatments or procedures;
      7. (vii) mental and psychosocial status;
      8. (viii) discharge potential;
      9. (ix) dental condition;
      10. (x) activities potential;
      11. (xi) cognitive status;
      12. (xii) drug therapy; and
   3. (c) a post discharge plan of care that is developed with the participation of the resident and family, which will assist the resident to adjust to the new living environment.
