1. (1) There is a one-time application fee of $25 for opening a new account. The fee must be paid when the new account is established.
2. (2) Financial institutions acting as program managers may impose fees upon account owners. Program management fees must be approved by the department before they can be imposed.
3. (3) Application fees collected must be remitted to the department and placed in a state special revenue account and only be used for administration of the program.
4. (4) Program management fees will be retained by each program manager as compensation for program management services.
