1. (1) An EDC must have written policies and procedures for determining eligibility for services that include:

   1. (a) the criteria to determine eligibility for services;
   2. (b) the information required to be collected to determine eligibility for services;
   3. (c) the population of individuals accepted or not accepted for services; and
   4. (d) the procedures for accepting referrals.
2. (2) The EDC must have a policy and procedures for managing wait lists for services.
