1. (1) For each employee or contracted individual, the outpatient crisis response facility shall maintain the following information on file:

   1. (a) a current job description;
   2. (b) if a licensed mental health professional, documentation of current licensure and certification; and
   3. (c) dated documentation of the individual's involvement in orientation, training, and continuing education activities.
