1. (1) The patient must be provided with an aftercare plan upon discharge.

2. (2) Each facility shall prepare a discharge plan for each patient no longer receiving services. The discharge plan must include:

   1. (a) the reason for discharge;
   2. (b) a summary of the services provided by the facility including recommendations for aftercare services and referrals to other services, if applicable;
   3. (c) an evaluation of the client's progress as measured by the treatment plan and the impact of the services provided by the facility;
   4. (d) diagnosis and response to medications; and
   5. (e) the signature of the staff member who prepared the report and the date of preparation.
3. (3) The discharge summary must be filed in the clinical record within 72 hours after patient is discharged from the BHIF.
