1. (1) Permits may be canceled by the department for cause including but not limited to the following:

   1. (a) violation of any permit condition;
   2. (b) failure to fully disclose all relevant facts or obtaining a permit through misrepresentation;
   3. (c) violation of the act or these rules; or
   4. (d) change or newly discovered condition or circumstance that makes or would make the dam unsafe.
