1. (1) When there is a change of the appointed attorney-in-fact, the certified euthanasia agency must:

   1. (a) comply with any DEA notification requirements concerning the change of the attorney-in-fact; and
   2. (b) complete a department power of attorney form appointing a new attorney-in-fact. The form must be submitted to the board within ten days of the change.
