1. (1) The high school transcript is the official academic record of the courses and credits completed by a high school student. At a minimum, the high school transcript must include:

   1. (a) statewide student identifier;
   2. (b) student legal name, address, gender, and date of birth;
   3. (c) statewide school identifier, school name, school address, school phone, school system name, school system code, and school code;
   4. (d) student grade level;
   5. (e) student entry date and exit date;
   6. (f) course code, course title, course grade, and credits earned for each school term and course taken;
   7. (g) grade point average;
   8. (h) graduation date;
   9. (i) diploma type;
   10. (j) class rank;
   11. (k) number in class;
   12. (l) for each test reported, the test date and the score for each portion of the test;
   13. (m) academic honors, if applicable; and
   14. (n) required number of credits for graduation.
