1. This form paragraph is only to be used in a nonprovisional international design application.

5. When the applicant has furnished the application title, applicant's authorization is required to make an examiner's amendment to the application title. See MPEP §§ 1302.04 and 2920.04(a). If an extension of time is required, use form paragraph 13.02.02 instead of form paragraphs 13.02 and 13.02.01.
