1. 1. The work certificate shall show

   1. (1) The age, sex, place and date of birth and place of residence of the child;
   2. (2) The name and place of residence of the child's parent, guardian or custodian;
   3. (3) The name and address of the employer; and
   4. (4) The nature of the employment for which the work certificate is issued.
2. 2. Work certificates authorizing the employment of a child during nonschool hours shall be of a form and color distinct from those authorizing full time employment.
