PURPOSE: This rule prescribes the forms adopted and approved for filing with the Office of the Attorney General.

1. (1) The following forms have been adopted and approved for filing with the attorney general’s office, trade offenses division:

   1. (A) Form 1-A: Health Spa Initial Registration Statement; and
   2. (B) Form 1-B: Health Spa Annual Renewal Registration.
