A certified recovery residence must maintain documentation with a resident's signature stating that each resident received the following prior to or on the first day of residency:

1. (1) the recovery resident bill of rights in section 254B.211, subdivision 3;
2. (2) the residence's financial obligations and agreements, refund policy, and payments from third-party payers for any fees paid on the resident's behalf;
3. (3) a description of the services provided by the recovery residence;
4. (4) relapse policies;
5. (5) policies regarding personal property;
6. (6) orientation to emergency procedures;
7. (7) orientation to resident rules; and
8. (8) all other applicable orientation materials identified in sections 254B.21 to 254B.216.
