1. 1. Minimum standards The following are minimum standards for an eye examination:

   1. A. A history of the patient's ocular and medical care;

   2. B. A record of the entrance visual acuity of each eye;

   3. C. A physical examination of each eye in an in-person clinical setting by the licensee in accordance with any requirements and restrictions imposed by this chapter and in accordance with the standard of care;

   4. D. An assessment of the examination results;

   5. E. A treatment and management plan;

   6. F. If performing a refraction, the performance of an objective and subjective refraction, when practicable; and
