Every provider shall provide to its subscribers within 120 days following the close of its first fiscal year of operation:

1. 1. Statement of financial condition The most recent certified annual statement of financial condition, including a balance sheet and summary of receipts and disbursements, including notes of that statement;

2. 2. Description of structure and operation A description of the organizational structure and operation of the provider, including the kind and extent of subscriber participation and a summary of any material changes since the issuance of the last report;

3. 3. Description of services A description of services and information as to where and how to secure them; and

4. 4. Method of subscriber complaints A clear and understandable description of the provider's method for resolving subscriber complaints.
