If commitment or a supervised living arrangement is ordered, the court shall require the head of the institutional facility or the person in charge of supervision to submit:

1. 1. Plan of treatment A plan of treatment within 10 days of the commencement of the commitment or supervision; and

2. 2. Written report A written report, with a copy to both the department and the individual, at least 20 days, but not more than 25 days, from the start of the commitment or supervision, setting forth the following:

   1. A. The types of support or therapy groups, if any, which the individual is attending and how often the individual attends;

   2. B. The type of care or treatment the individual is receiving and what future care is necessary;

   3. C. Whether the individual has been cured or made noninfectious or otherwise has ceased to pose a threat to public health;

   4. D. Whether continued supervision or commitment is necessary; and

   5. E. Any other information the court considers necessary.
