An applicant shall:
- A. Furnish proof of service or proof of spouse's service;
- B. Complete an application form that provides information about the applicant's personal history and financial status;
- C. Enclose a copy of honorable discharge papers;
- D. Submit an income affidavit;
- E. Have the results of a physical exam documented by the applicant's physician on the application form; and
- F. Provide the medical director with medical records if the applicant has been hospitalized.
