The affidavit shall include:

1. A. The name and account number of the campaign finance entity;

2. B. A statement under penalties of perjury that, since the establishment of the campaign finance entity or since the last filed campaign finance report prescribed by Election Law Article, §13-309, Annotated Code of Maryland, the campaign finance entity has not:

   1. (1) Received contributions in the cumulative amount of $1,000 or more; and
   2. (2) Made expenditures, exclusive of the candidate's filing fee, in the cumulative amount of $1,000 or more; and
3. C. An acknowledgment that the campaign finance entity is required to either file the next scheduled campaign finance report or file a new affidavit in lieu of that report.
