A commercial ambulance service shall maintain:

1. A. A complete and current personnel list which includes:

   1. (1) EMS providers;
   2. (2) Ambulance drivers;
   3. (3) Registered nurses; and
   4. (4) Any other medical personnel employed;

2. B. Personnel lists which include each employee's:

   1. (1) Full name; and

   2. (2) Health care certification or license:

      1. (a) Number,
      2. (b) Level,
      3. (c) Date of issuance, and
      4. (d) Date of expiration;

3. C. Accurate and completed employee training records for:

   1. (1) New employee orientation;
   2. (2) Continuing education; and
   3. (3) All training required for compliance with this subtitle;

4. D. Training records that document:

   1. (1) Date training provided;
   2. (2) Course outline;
   3. (3) Employee attendance;
   4. (4) Instructor name; and
   5. (5) Instructor's qualifications; and

5. E. Accurate records of employee:

   1. (1) Communicable disease exposure histories;
   2. (2) Immunization records; and
   3. (3) Yearly TB screening.
