A commercial ambulance service shall:

1. A. Maintain records in any permanent form including:

   1. (1) Paper, or
   2. (2) Magnetic media;

2. B. Organize all records in such a manner as to allow the commercial ambulance service to locate, within a reasonable time, a record, given:

   1. (1) A patient's name,
   2. (2) Date and time of a call, and
   3. (3) Pick up or discharge location; and

3. C. Retain all records for not less than 5 years and in accordance with:

   1. (1) Local law,
   2. (2) State law, and
   3. (3) Federal law.
