1. A. Except as otherwise provided in this chapter, notice shall be by certified and regular mail.

2. B. Notice to an EMS provider shall be mailed to:

   1. (1) The address the EMS provider maintains for purposes of licensure or certification notice; or
   2. (2) Any other last known address.
3. C. Notice to any other person shall be to the person's last known address.
