1. A. A petition for Declaratory Ruling shall be filed in writing and clearly marked to indicate that it is being filed pursuant to this regulation. It shall describe in detail:

   1. (1) The interest of the petitioner in making the request;
   2. (2) The issues involved;
   3. (3) A statement of the facts;
   4. (4) A list of documents or statements the petitioner believes should be considered; and
   5. (5) A sworn statement by the petitioner that the facts contained in the petition are true to the best of the person’s knowledge and belief.

2. B. A petition for Declaratory Ruling shall include the petitioner’s:

   1. (1) Name;
   2. (2) Address;
   3. (3) Telephone number;
   4. (4) E-mail address; and
   5. (5) Facsimile number, if available.
