1. A. The record-keeping provisions of Regulation .05D of this chapter specify that an owner or operator shall keep a written operating record at the facility. This regulation provides additional instructions for keeping portions of the operating record. See Regulation .05D(2) of this chapter for additional record-keeping requirements.

2. B. For each hazardous waste received, treated, stored, or disposed of at a facility, the owner or operator shall record the following information in the facility's operating record and maintain this information in the operating record until closure of the facility:

   1. (1) A description of the waste, including:

      1. (a) The common name of the waste and the EPA Hazardous Waste Number or Numbers from COMAR 26.13.02 which apply to the waste,
      2. (b) The waste's physical form, such as liquid, sludge, solid, or contained gas,
      3. (c) If the waste is not listed in COMAR 26.13.02.15 — .19, a description of the process that produced it, such as “solid filter cake from production of _________, EPA Hazardous Waste Code W051”, and
      4. (d) The EPA or State Hazardous Waste Number from COMAR 26.13.02.10—.19 that is associated with the waste, or, if the waste contains more than one listed hazardous waste or when more than one hazardous waste characteristic applies to the waste, all applicable EPA or State Hazardous Waste Numbers;
   2. (2) The estimated or manifest-reported weight, or volume and density, when applicable, in one of the units of measure specified in Table 1; and

   3. (3) The method or methods (by handling code or codes as specified in Table 2) and date or dates of treatment, storage, or disposal. Table 1
