1. A. Application Completion Criteria. Applications for a scrap tire hauler license are considered complete only if they are:

   1. (1) Submitted on application forms provided by the Department;
   2. (2) Accompanied by all required exhibits and supporting documents, as applicable; and
   3. (3) Completed in full and signed by the applicant.

2. B. Upon receipt of a completed application, the Department may deny the license if the:

   1. (1) Application contains a material misrepresentation or false information, or fails to fully disclose relevant facts; and
   2. (2) Applicant has not complied with these regulations, or other applicable statutes and regulations of the Department or other governmental agencies.
