Whenever a party is required by this chapter to serve a copy of a pleading or memorandum upon another party, a certificate shall be appended to the pleading or memorandum stating:

1. A. To whom the copy was sent;
2. B. The address to which the copy was sent;
3. C. The name of the person who sent the copy;
4. D. The date the copy was sent; and
5. E. Manner of service.
