Published or circulated notice of the hearing shall contain:

1. A. The time of the hearing, if required;
2. B. The place of the hearing, giving the street address, floor, and designated room;
3. C. The purpose of the hearing, in clear, intelligible language;
4. D. If necessary, reference to the particular statute which gives authority for the hearing and the proposed regulation.
