1. A. In a contested case all parties shall be afforded an opportunity for hearing after reasonable notice.

2. B. The notice shall:

   1. (1) State the time, place, and nature of the hearing;
   2. (2) State the legal authority under which the hearing is to be held;
   3. (3) Cite the particular sections of the statutes and regulations, including procedural regulations, that are pertinent; and

   4. (4) State concisely and simply:

      1. (a) The facts that are asserted; or
      2. (b) If the facts cannot be stated in detail when the notice is given, the issues that are involved.
