1. A. Inspections.

   1. (1) Except as provided in §A(2), of this regulation, the custodian shall make available for inspection, without a written application, any record that clearly is required to be disclosed.

   2. (2) The custodian may require a written application if the custodian reasonably believes that:

      1. (a) A written application will materially assist the Office in responding to the request;
      2. (b) The Act or any other law may prohibit disclosure of the record; or
      3. (c) The Act may authorize nondisclosure.
2. B. Copies. If the applicant is requesting a copy of a public record, the custodian may require a written application.

3. C. Contents of Application. A written application shall contain an applicant's:

   1. (1) Name;
   2. (2) Address; and
   3. (3) A brief description of the record sought.
