The owner shall include in each direct billing of energy costs to the occupant the following information:

1. A. Bill date (the date the bill was prepared);
2. B. Time period covered by the bill (billing period);
3. C. Readings of the measurement device at the beginning and at the end of the billing period;
4. D. Number of the measurement units recorded by the measuring device;
5. E. Approximate energy use;
6. F. Per unit energy charge;
7. G. Total energy costs billed to the occupant;
8. H. A statement that the bill is not from the utility; and
9. I. A local or toll-free telephone number for billing inquiries.
