1. A. In General.

   1. (1) Except as provided in §A(2) of this regulation, a request to inspect a public record shall be made in writing.

   2. (2) A request need not be in writing if:

      1. (a) The record is of a type listed by the Secretary under Regulation .04B of this chapter; or
      2. (b) The custodian waives the requirement for a written request.

2. B. Contents of Written Request. A written request shall:

   1. (1) Reasonably identify, by brief description, the record sought;
   2. (2) Contain the applicant's name, address, and telephone number, as needed for the purpose of contacting the applicant; and
   3. (3) Be signed by the applicant.
3. C. Filing Written Request. The applicant shall address a written request to the custodian of the record sought.
