1. A. The permittee shall generate a report containing the following information and maintain all documents used to generate the report for a period of 5 years:

   1. (1) The source, type, and quantity of food processing residuals transported;
   2. (2) The dates of transportation of food processing residuals;
   3. (3) The destinations of food processing residuals (i.e., the address of the agricultural operation);
   4. (4) Other related information regarding the transportation of food processing residuals as required by the applicable Food Processing Residuals Utilization Permit; and
   5. (5) Other information that may be requested by the Department.
2. B. The permittee shall submit semi-annually to the Department two completed copies of the report required by §A of this regulation for each year the Food Processing Residuals Utilization Permit is in effect. The permittee shall submit to the Department the report on a form provided by the Department.
