A qualified applicant who applies for a grant shall apply on a departmental form that includes, among other things, the following:

1. A. The project title;
2. B. The applicant’s name and contact information;
3. C. The qualifications of the applicant and the personnel who will work on the project, including veterinary services, if applicable;
4. D. The amount of the grant requested; and

5. E. A project description, which shall include:

   1. (1) The project’s objectives and expected outcomes;
   2. (2) A work plan and budget summary of the proposed project;
   3. (3) How the project meets the purposes of the Fund and the requirements of the grant; and
   4. (4) Any additional information required by the Department.
