The petition shall be in writing and shall contain:

1. A. A clear, concise statement of the regulation, amendment, or repeal which is sought;
2. B. Reasons in support of the proposed regulation change;
3. C. References to the authority of the Authority to take the action which is requested; and
4. D. The name, address, and telephone number of the petitioner.
