1. A. Unless otherwise required by law, service of the following documents shall be made by personal delivery or by mail:

   1. (1) Notices of the hearing;
   2. (2) Default orders;
   3. (3) Prehearing orders;
   4. (4) Final decisions; and
   5. (5) Exceptions to proposed decisions and appeals of final decisions.
2. B. Service upon a party to whom a permit, certification, or license has been issued by the Commission or Secretary may be made by delivering a copy to the most recent address provided by that party to the Commission or Secretary.

3. C. Proof of Service. The person serving a document shall certify:

   1. (1) To whom the document was sent or delivered;
   2. (2) The address to which the copy was sent or delivered;
   3. (3) The date the copy was sent or delivered;
   4. (4) The manner of service; and
   5. (5) The name of the person who sent or delivered the document.
